Incident Report

Report a Code of Conduct Violation

We are committed to creating a safe, respectful, and positive environment for all players and parents. If you witness any behavior on the field that violates the Player or Parent Code of Conduct, please report it to the Palos AYSO Regional Board of Directors.

To report an incident:

• Include details such as the date, time, and location of the incident.
• Provide the names of individuals involved (if known).
• Describe the nature of the violation.
